4me.md is owned and operated by Christian Medical PLLC. Christian Medical PLLC operates the 4me.md website and associated services. This Privacy Policy describes how Christian Medical PLLC, through 4me.md, collects, uses, maintains, protects, and discloses information obtained through the website and services.
Information we collect
Depending on how you use the service, we may collect identifying and contact information, health information you choose to submit, intake answers, secure care-room messages, selected pharmacy information, communication preferences, consent records, device and security information, and records needed to operate and protect the service.
How we use information
We use information to review care requests, communicate about intake and scheduling, provide requested services, operate and secure the clinician portal, prevent fraud and abuse, maintain required records, comply with law, and improve service reliability.
Text messaging
If you expressly choose text messaging and check the SMS consent box, 4me.md may send non-marketing messages about your intake, scheduling, and care support. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Choosing not to receive text messages will not prevent you from requesting care by another available method.
SMS consent and mobile information are not sold or shared with third parties or affiliates for marketing or promotional purposes. Information may be disclosed to service providers that help deliver requested communications, subject to applicable safeguards and legal requirements.
Secure care room and video
Care-room messages are encrypted and made available to the patient and authorized clinical users. Live video is transmitted through an AWS-hosted meeting service and is not recorded by the 4me.md care-room feature. Technical metadata may be processed to establish, secure, troubleshoot, and document the communication. Protect the unique care-room link as you would a password.
How information may be disclosed
Information may be disclosed to authorized clinicians and workforce members, service providers supporting secure hosting, communications, security, payment or operations, a selected pharmacy when necessary and authorized, and government or other parties when required or permitted by law. We do not sell patient information.
Security and retention
We use administrative, technical, and physical safeguards designed to protect information. No internet or messaging system can be guaranteed completely secure. We retain information only as long as reasonably necessary for care, operations, legal obligations, security, and dispute resolution.
Your choices
You may opt out of text messages by replying STOP. For help, reply HELP, call or text 469-931-3614, or email doctor@4me.md. You may also contact us about access, correction, or other privacy questions, subject to applicable law.
Children and emergencies
A parent or legal guardian should submit information for a minor when required. This website is not an emergency service. Call 911 or go to the nearest emergency department for an emergency.
Changes to this policy
We may update this policy. The updated date above identifies the current version. Continued use after an update is subject to the revised policy.
