4me.md is a website and service owned and operated by Christian Medical PLLC (“Christian Medical,” “4me.md,” “we,” “us,” or “our”). Christian Medical PLLC is the legal business entity responsible for the 4me.md website and associated services. These Terms govern use of the 4me.md website, intake forms, secure care room, video visits, clinician portal, telephone services, and text-messaging services. By using the service, you agree to these Terms and our Privacy Policy.

Not an emergency service

Do not use 4me.md for an emergency. Call 911 or go to the nearest emergency department. Online submissions, secure messages, and text messages are not monitored continuously and do not guarantee an immediate response.

Care requests

Submitting an intake form does not guarantee treatment, a prescription, a diagnosis, or establishment of a clinician-patient relationship. A clinician must review the request and determine whether remote care is appropriate. You agree to provide complete and accurate information and to seek in-person care when advised.

Fees and payments

Urgent-care asynchronous review and secure online messaging each cost $25; an urgent-care live video evaluation costs $35. The initial GLP-1 evaluation costs $49 whether completed asynchronously, by secure online messaging, or by live video. Payment does not purchase or guarantee a prescription, medication, diagnosis, eligibility for a GLP-1 medication, or any particular medical outcome.

If a patient is medically accepted into the GLP-1 program and elects to enroll, ongoing subscription pricing is dose-based. Current monthly tiers are: semaglutide 0.25 mg or 0.5 mg — $169; 1 mg — $189; 1.7 mg — $229; 2.4 mg — $269; clinician-directed flex dose — $319. Tirzepatide 2.5 mg — $219; 5 mg — $269; 7.5 mg — $319; 10 mg — $349; 12.5 mg or 15 mg — $379. The subscription price includes ongoing physician management, prescribed program medication when medically appropriate, and standard overnight medication shipping. Laboratory testing, additional live video follow-ups, or other outside services, when needed, are separate and disclosed before ordering.

GLP-1 monthly management and consultation services renew monthly until canceled. During enrollment, the patient separately authorizes 4me.md to use the payment method used for the initial consultation for recurring monthly professional-service charges at the disclosed monthly fee. Recurring billing is not activated unless the patient has provided that authorization and a clinician has approved ongoing care. The recurring 4me.md charge is for telehealth consultation and clinical management services only and does not include medication, pharmacy, laboratory, or other third-party charges. Patients may cancel future renewals before the next billing cycle.

Cancellation stops future renewals but does not reverse services already provided, medication already ordered or dispensed, or charges already earned or incurred, except where required by law. The $49 initial-evaluation fee is separate from recurring monthly subscription charges.

Payments are processed through Stripe’s hosted payment services. Card information is entered through Stripe and is not stored directly by 4me.md. 4me.md stores only payment-provider identifiers and billing references needed to administer authorized payments and recurring professional-service billing.

GLP-1 medications

Medication is prescribed only when a licensed clinician determines that treatment is medically appropriate and any patient-specific basis required for a compounded prescription is documented. Patients may state a preference, but they do not order or select a vial, concentration, quantity, or dose. The clinician selects the medication and regimen and sends a patient-specific prescription to the dispensing pharmacy. Medication selection, formulation, dose, availability, and dispensing pharmacy may change based on the patient’s clinical circumstances, applicable law, and pharmacy availability. Compounded medications are not FDA-approved and are not the same as FDA-approved branded products. No particular medication or dose is guaranteed.

Asynchronous, secure-message, and video care

Depending on the service and clinical circumstances, care may be provided through asynchronous clinician review, secure online messaging, or live video. A clinician may require video, telephone, testing, or in-person evaluation before making a treatment decision. Asynchronous and secure-message services are not real-time services. Video visits require a compatible device, camera, microphone, reliable internet access, and a private location. 4me.md does not record live video visits through the care-room feature.

The unique care-room link issued after payment is a confidential access credential. You are responsible for protecting it and should not forward or publish it. Contact 4me.md promptly if you believe the link has been exposed.

Text-message terms

If you choose text messaging and provide express consent, you may receive non-marketing messages from 4me.md about intake confirmation, scheduling, and care support. Message frequency varies. Message and data rates may apply. Carriers are not liable for delayed or undelivered messages.

• Consent to text messaging is not a condition of receiving care.
• Do not send urgent or highly sensitive medical information by ordinary text message.

Acceptable use

You may not misuse the service, attempt unauthorized access, submit false or unlawful information, interfere with security, impersonate another person, or use the service in a way that violates law or the rights of others.

Availability and third-party services

Service availability may change and may be interrupted. Features may rely on third-party hosting, communications, mapping, pharmacy, or security providers. References or links to third-party services do not constitute a guarantee of those services.

Disclaimers and limitation

The website and administrative features are provided on an as-available basis to the extent permitted by law. Nothing in these Terms limits rights or responsibilities that cannot legally be limited, including applicable professional obligations.

Changes

We may update these Terms. The updated date above identifies the current version. Continued use after an update is subject to the revised Terms.
